Carer Support Service at New Court Surgery

Are you a carer for a relative, friend or neighbour who could not manage without your help?

New Court Surgery & Worcestershire Association of Carers are providing this service dedicated to the support of carers. Carers can make an appointment with the GP Carer Support Adviser, who can meet them at the practice or at home if the carer is currently unable to leave the person they care for on their own. The support service is designed to reach out to carers, telling them about the support available to them and looking at numerous issues including social isolation, accessing correct benefits, respite care etc.

Sophie Wood is the GP Carer Support Adviser for New Court Surgery.

Would you like a one to one appointment to discuss support available to you as a carer?

Please ask reception or your GP for a referral. Alternately you can call the GP Carer Support Team directly on: 01905 751349
